For me, the lyrics are the thoughts of a person who truely believes that they are doing something good for other people, with no expectation of a physical reward. i.e. they are only doing it to help other people.
however. this spin on this is that they are in fact doing it for a reward. for the feeling that they get from doing it. Obviously, in this example they are giving blood, possibly the most common selfless act. they have someone their helping them through it, telling them how selfless they are, when really that is their own reward.
