it might have been more times than that, since not every show might have been listed in their "tour journal" on that site

you can even click there to find out what dates it was played on also

I know they played it July 14, 1998 in Akron, Ohio, on the Ozzfest Tour. It was the show opener. I've heard a good bootleg of it.

Forgot to add: Maynard was in good "reverand" form that show. After Sweat and before Stinkfist he took a moment to "introduce the players." He introduced them as "Daniel the lion-fucker Carey ... Justin Chancelor, unfortunately, a Jew ... and on guitar, Adam, the right hand of Eve, Jones." It was funny.
